​Scholarship & Award Appeal Form

For students requesting reconsideration of a scholarship or award decision due to extenuating circumstances.
Appeals may be submitted for situations such as:

• Changes in enrollment
• Medical circumstances
• Military deployment
• Other documented extenuating circumstances

Appeals are reviewed by the Scholarship Committee on a monthly basis.
